Amber Cotton Nutrition is registered with the Complementary and Natural Healthcare Council (CNHC) and a member of the professional association, British Association for Nutrition and Lifestyle Medicine (BANT).
Clients Under the Age of 18 Years
Amber Cotton Nutrition will not advise or consult with clients under eighteen years of age without their parent/guardian present, or without written consent from the child’s parent/guardian for another adult to attend or for the child to attend part of the consultation alone.
Tolerance Policy
A healthy practitioner/client relationship is based on trust and mutual respect. On the rare occasion that this relationship may break down, Amber Cotton Nutrition may decide that it is in the best interests of the client that he or she finds a new practice. Equally, if a client refuses to comply with clinic policies, we may have no option other than to refuse any further treatment.
No form of abuse, be it physical, sexual or verbal, will be tolerated and will result in immediate removal from our client base.
Conditions of Practice
The Client should note the following conditions of practice:
The degree of benefit obtainable from nutritional therapy may vary between clients with similar health problems and following a similar nutritional therapy programme.
Nutrition and lifestyle recommendations will be tailored to support health conditions and/or health concerns identified and agreed between both parties.
Amber Cotton Nutrition is not permitted to diagnose, or claim to treat, medical conditions.
Amber Cotton Nutrition’s recommendations are not a substitute for professional medical advice and/or treatment.
Amber Cotton Nutrition may recommend food supplements and/or functional testing as part of your nutritional therapy programme, and may receive a commission on these products or services.
Standards of professional practice in nutritional therapy are governed by the Complementary and Natural Healthcare Council (CNHC) Code of Conduct, Ethics and Performance.
Amber Cotton Nutrition and the Client are entering into an agreement in which Amber Cotton Nutrition will deliver services as a Nutritional Therapist and Health Coach. No psychological counselling or therapy will be provided within your nutritional therapy programme. If these are needed it is the Client’s responsibility to seek these services from a licensed professional.
The Client will need to understand and agree to the following:
I am responsible for contacting my GP about any health concerns.
If I am receiving treatment from my GP, or any other medical provider, I should tell him/her about any nutritional recommendations provided by my registered nutritional therapy practitioner. This is necessary because of any possible reaction between medication and the nutritional programme.
It is important that I tell my registered nutritional therapy practitioner about any medical diagnosis, medication, herbal medicine or food supplements I am taking as this may affect the nutritional programme.
If I am unclear about the agreed nutritional therapy programme/food supplement doses/time period, I should contact my registered nutritional therapy practitioner promptly for clarification.
I understand that the recommendations are personal to me and may not be appropriate for others.
I must contact my registered nutritional therapy practitioner should I wish to continue any specified supplement programme for longer than the originally agreed period, to avoid any potential adverse reactions.
Recording consultations using any form of electronic media is not allowed without the written permission of both me and my registered nutritional therapy practitioner.
It is my responsibility to ensure I have a working internet connection for online appointments. If I am unable to connect as scheduled, the appointment is considered a ‘no show’ and charged in full.
Confidentiality and Data Protection
Amber Cotton Nutrition will keep your personal information confidential and secure following the UK GDPR guidelines for the practice. Amber Cotton Nutrition will not share your information with third parties without your consent. However, if Amber Cotton Nutrition believes there is a risk of significant harm to yourself or another person, Amber Cotton Nutrition may pass the information onto an appropriate authority using the legal basis of vital interest. A separate Privacy and Consent Notice is available for your review.
Payment Terms and Cancellation Policy
Package fees must be paid in full 7 days in advance of your first face-to-face or online consultation unless otherwise agreed with Amber Cotton Nutrition. Payments can be made by bank transfer, credit or debit card.
All sessions require pre-payment and will be cancelled by Amber Cotton Nutrition if this has not been taken care of in advance.
I reserve the right to change the fees offered but will always honour the agreed rate for the agreed time. Should you decide to continue working with Amber Cotton Nutrition there is no guarantee that the rates will be the same.
All programmes must be started and completed within 16 weeks from the scheduled date of the first consultation. After this time you will forfeit the right to claim a refund.
Appointments cancelled with less than 24 hours’ notice and ‘no shows’ will be charged in full.
In the case of advance payments for services, you have the right to cancel this Agreement within 14 days of the purchase without penalty (‘Cooling-off Period’). If you wish to terminate the agreement within the 14-day Cooling-off Period, you must do so in writing to amber@ambercotton.co.uk. Amber Cotton Nutrition will refund any monies paid prior to cancelling this Agreement. If the services have commenced during the 14-day Cooling-off Period, Amber Cotton Nutrition will refund any monies due to you for services not yet provided. The refund payment will be made within 14 days of cancellation. Refunds are not given after the ‘cooling off’ period. A credit note will be offered instead where the first consultation must be taken within 6 months of the issue date. The programme expiry date of 16 weeks will commence on the date of the initial consultation.
